Our Financial Policy

Please take a moment to read our financial policy. We are committed to having a successful doctor/patient relationship. Please understand that paying your bill is part of that relationship.

Full payment is expected at the time of service, unless other arrangements have been made. The office accepts credit cards (Mastercard, Visa, American Express and Discover) as payment for medical services.  Charges for services are non refundable;  however, in the event of overpayment, a credit will be issued within 30 days.

Insurance: The office accepts Wellfleet Insurance (for NYU Students), Aetna Student Health Insurance and serveral other Aetna plans

Some insurance plans, including Aetna Student Health, require a referal to see a specialist. If a referal is necessary, but not obtained, then the patient will be responsible for the full cost of the visit.

Patients provide a copy of their insurance card before the first visit. The office will bill the insurance company directly. However, Co-payments are due at the time of service.

If you have an insurance which we do not accept, we will be happy to provide you with a Health Insurance Claim Form (HICF) to submit for "out of network" reimbursement. In such a case, the patient will be reimbursed directly, and full payment is expected at the time of service.

Finally, the office requires 24 hour notice for all scheduled appointments. Patients will be responsible for the full cost of the visit, if adequate notice is not given. Also note, missed visits can not be billed to your insurance company.

All billing and insurance questions should directed to the office via email (michelle.cervone@gmail.com) or securely through this site.
